Own Icon not showing in explorer or startup menu

Icons made in VS05 for a C++ Application don't show in all places.
Instead I see the MFC Icon. When viewing the file name in explorer or in
the startup menu I see the MFC Icon. Everywhere else the Icon is shown
correctly.

I looked in many places for help but have so far not come up with a
solution to the problem.

Have you edited all the different size/resolutions of the icon? A
single icon can have multiple images at different sizes and colour
depths.
