Simon,
I found this Microsoft Knowledge Base Article that sounds like your issue.
It appears that if you rename the SQL Server, and the SQL Server Name and
the Computer Name are different, then it will not step into the debugger.

Hi,
Both the computer name and the server name have not changed and are the
same. I've still tried this without much success.
