Close all IDE instances, open a VS.NET command prompt an type:
devenv.exe /setup
That should reset the environment.

I have the same problem and it has happened multiple times for me as well. I
did a 'devenv.exe /setup' which took only an instant to run, but upon opening
visual studio again the duplicate menus still exist.
I also tried removing my profile, resetting the toolbar (via the customize),
and doing a repair install, none of which were able to fix the problem.
I fixed it before by uninstalling and reinstalling. Any other tips so I can
save the hours of uninstalling, reinstalling and service pack installing.
