Hi. You're right that, for example, my Smart Phone 2003 has a little
joy-stick thing but no stylus. Yet, I'm confused about this: If I want to
have an interface where the user can move between "hot-spots" (such as a text
link, a button, etc.) and then press the center button to effectively execute
a "click," how is one supposed to do this in a Smartphone 2003 .NET
application?

I think there must have a workaround to this scenario, but I am not an
experienced mobile device developer, I am afraid I cannot be of more help
help. I suggest you can ask this in a more appropriate newsgroup or MSDN
forum, you'd get better and quicker help there:
