There is another circumstance that can cause the same effect: if add-ins
toolbars share the same row. See the whole discussion:

Hi Carlos,
Thank you very very much. My problem is the combination of Toolbar and
CommandBar issues discussed in the links that you provided. I am now working
on a solution.
I do appreciate you finding the time to point me to the answer. I have been
