Are you using "roaming documents", where your "My Documents" folder is actually located on a network server rather than your local machine?  We have seen some similar issues connected to this before.

I installed the DevExpress packages, and restarted VS, and all the tabs are present.

Next day, start VS, only default toolbox items. If I right-click and choose "Show All", they appear, but all items are grayed out.

Next day, start VS, only default toolbox items, show all doesn't show anything but default.

This is a laptop that I use both when I'm connected to a network and when I'm not, and occasionally when I start VS it says something about configuring for first use, could this be the problem ?

If so, is there a way to force it to never do this again ? or make it remember the settings ?

Yes, that was the problem. I ended up disabling the whole offline folder
part and moving the My Documents folder back onto the C: disk and
everything is A-Ok now.
