I've tried ndoc to create chm help files from the xml files produced by
visual studio.  It's pretty good, but I'm wondering is there other
utilities, perhaps microsoft ones, that do the same thing?

Currently, there are no Microsoft tools for this, unless the newest HTML help
can consume these XML files. As nDoc is open source, you have the ability to
download and alter the code to fit your needs (realizing this will stop you
from being able to easily download any bug fixes).
